CREDIT ROLLOVER POLICY

Monthly payments become treatment credit.

Credit Expiration

Unused credits roll over for up to:

6 months while membership remains active

After Cancellation

Recommended:

Credits expire 30 days after cancellation

CHARGEBACK PROTECTION TERMS

Billing Authorization

Member authorizes recurring monthly autopay.

Non-Refundable Clause

Membership fees are non-refundable once processed.

Chargeback Clause

suspend membership

 recover pricing differences

 recover collection costs

 terminate benefits for improper disputes
